The Ministry of Heath of The Bahamas requires that the Application to Travel form and Consent for Inter-island Travel & Entry Into The Bahamas form be completed by all persons requesting permission to travel to or within The Bahamas.
These forms must be completed prior to travel and emailed to: COVID19travel@bahamas.gov.bs
